New Patient Information
Everything you need to know before your first visit, from what to bring and what to expect, to forms, policies, and insurance information.
Your first visit, step by step
We want your first appointment to be productive, not stressful. Here's exactly what to expect, from scheduling through your first consultation.
Get a referral from your primary care physician
All new patients to The Endocrinology Group require a referral from a clinician stating the reason for consultation. This may include labs, imaging, or clinician notes. Some insurance plans also require a separate insurance referral, which must be received prior to scheduling. Please check with your referring clinician to confirm what is needed — our staff will verify that all referrals are on file before your appointment is scheduled.
Schedule your new patient appointment
Call our office at (571) 634-3636 — this number serves all locations, regardless of which office you'd like to be seen at. You may also request an appointment online. Let us know the reason for your visit so we can match you with the right clinician and allocate enough time.
Complete new patient forms
Download and complete your new patient forms and submit them through the patient portal before your visit. If you are unable to complete the forms online, you are required to arrive 30 minutes before your scheduled appointment to fill them out in office and avoid delays to your visit.
Gather your records and device data
Bring recent lab results (especially A1C, thyroid function, and lipid panels), relevant imaging (such as thyroid ultrasound or DEXA scans), a current medication list with doses, and any CGM or insulin pump data if applicable.
Arrive early
New patients should arrive 30 minutes early; established patients should arrive 15 minutes early to complete check-in and verify insurance. Please note: patients who arrive more than 10 minutes late may not be seen, at the practice's discretion.
Your consultation — typically 40 minutes
Your clinician will review your history, ask about your symptoms and goals, and conduct a physical examination. A physical exam is performed for both in-office and virtual visits. You'll leave with a clear care plan and next steps.
What to Bring
Please have these ready for your first appointment
- Insurance card & photo ID
- Current medication list with doses
- All recent labs and imaging studies
- List of current concerns or questions
- CGM or insulin pump data (if applicable)
- Completed new patient forms
- Names of other treating physicians
Complete these before your visit
Download and complete all applicable forms before your appointment. Bringing completed forms helps us spend more time on your care — and less on paperwork.
Important: If you are unable to download and complete your forms in advance, you are required to arrive 30 minutes before your scheduled appointment time to fill them out in office. Please be aware that delays in completing forms may result in a shorter appointment or the need to reschedule.
Patient Forms
Prefer to fill out forms online? You can also access and submit forms through the myPrivia patient portal. Log in or create an account to complete your intake paperwork before your appointment.
Our policies at a glance
We ask that all patients review these policies before their first visit. They're designed to help us provide consistent, high-quality care to everyone we see.
- Appointment Cancellations
We ask for at least 24 hours' notice if you need to cancel or reschedule. This allows us to offer the appointment to another patient. Late cancellations or no-shows may be subject to a fee. You can cancel or reschedule via the patient portal or by calling our office. - Prescription Refills
Prescription refill requests should be submitted through the patient portal or by calling our office. Please allow at least 3–5 business days for processing. We do not process urgent refill requests without a prior established relationship. - After-Hours & Urgent Calls
Our office is open Monday through Friday. For non-urgent questions, please use the secure patient portal messaging system. For urgent after-hours medical concerns, please call our main office line — after-hours calls are triaged through our answering service and will be subject to a virtual visit billed to your insurance. Privia Health also offers a [24/7 Virtual Clinic] for after-hours care needs. For emergencies, call 911. - Referral Policy
We are a subspecialty practice. We do not serve as primary care physicians and do not manage conditions outside of endocrinology. Patients are expected to maintain an active relationship with a primary care provider who coordinates their overall care. - Medical Records
Medical records requests are processed through Privia Health. Please allow up to 10 business days for processing. Requests can be submitted through the patient portal or via the Privia Health medical records request form. Please note that some requests may incur a fee depending on the type of records requested.
Insurance, billing & payment
We accept most major insurance plans. Here's what you need to know about coverage, billing, and payment options before your first visit.
- Insurance Accepted
We accept most major commercial insurance plans, Medicare, and plans in the Privia Health network. Coverage and copays vary by plan. If you have questions about whether your insurance is accepted, our staff are happy to assist — please don't hesitate to call our office. - Verify Before You Visit
Call your insurance provider to confirm that your plan is accepted and to understand your specialist copay or deductible responsibilities. Please note: all new patients require a clinician referral stating the reason for consultation (this may include labs, imaging, or clinical notes). Some insurance plans additionally require a separate insurance referral, which must be received before scheduling. Our staff will verify that both types of referrals are on file prior to your appointment. - Payment Options
We accept cash, check, and all major credit cards. Copays and patient balances are due at the time of service. Please be aware that failure to provide payment may result in the need to cancel or reschedule your appointment. For questions about your bill, please contact our billing department. - Uninsured Patients
We believe everyone deserves access to quality endocrine care. If you are uninsured or underinsured, please call our office to discuss self-pay rates and available options.
