There are two types of Referrals:  

  1. All patients require a Physician Referral stating reason for endocrinology consultation prior to scheduling, regardless of insurance.
  2. An Insurance Referral may also be required, though this varies by plan.  For coverage purposes, this must be received prior to scheduling.

Please arrive 30 minutes early to finalize new patient documentation and prepare for your consultation with the provider. Remember to bring the following:

  • Picture ID
  • Insurance Card(s)
  • Medication List
  • List of Specialists (if applicable)
  • Any questions for your provider (written down), lab results and imaging reports.
  • Glucometer and meters
