Billing & Insurance
We participate with most major insurance companies as listed below and will submit claims to your carrier on your behalf. If we do not participate with your insurance, we would be happy to provide you the paperwork necessary to submit for out-of-network benefit reimbursement through your insurance. Please be aware that in this situation, payment for services rendered will be expected at the time of your visit.
Some insurance companies require you to use a specific laboratory or to obtain referrals or pre-authorizations for office visits, testing, and treatment. It is the patient's responsibility to obtain the referrals for all office visits to avoid fees incurred should insurance deny coverage. If you need assistance with your insurance requirements, our staff will make every effort to help you.

We do not currently participate with:
United Healthcare plans (MAMSI, Optimum Choice, Mamsova), GreatWest, HealthKeepers Plus, Medicaid, Amerigroup.
It is always the patient's responsibility to confirm with their insurance companies that our participation agreements have not changed.
